About the role

As an Active Support Coordinator, you will proactively manage a portfolio of severely injured clients who are at varying stages of their rehabilitation in the community setting, aligning the provision of medical, rehabilitation and disability services, with the client's individualised plan to achieve maximal independence and participation in all life areas.
This role involves travel for face-to-face visits with clients and, their treating teams; therefore, travel is a required component of the position.  

About the Team

This role will be part of our Independence Branch, within the TAC. You will be supported by a manager who is open and flexible in their approach, your wellbeing and ability to succeed is paramount to the team's overall success.  You will join a team of Active Support Coordinators that are passionate about ensuring the best results for TAC clients, their families, treating teams and each other.

  • You are skilled in using a range of digital tools to support workload management, decision making, claims management and quality documentation
